Assistance Request Form (ARF)

Calaveras Health and Human Services Agency wants to make it easier for you to get help through the Unite Us referral system. This form connects you to the right support, either from our agency or from trusted community partners. Your answers help us understand your needs so we can connect you faster. We only share your information with partner organizations, and only with your permission. These partners may offer services like housing help, food programs, and health care.

Please note: being referred does not guarantee you qualify for the service, and some programs may have limited funding.
